ABOM Diplomate Certificate Replacement Request

Active Diplomates may request a replacement ABOM Certificate by filling out the form below and paying a replacement certificate fee of $100.

Please note the following:

  • The intent of a replacement certificate is replace a lost certificate.
  • Current signatory officers will be used on the replacement certificate.
  • ABOM will gladly postal mail the certificate anywhere in the United States or email them in the case of an international request. We do not postal mail to international addresses.
  • The purchase of a replacement certificate does not in any way waive the need to recertify, fulfill CE credits, or pay annual dues/fees.
  • A payment must be made before a certificate is created and sent to you. After payment is received, we will contact you within a week to confirm status of your certificate.
